Professional Survey of Wisconsin Athletic Trainer Resource Awareness and Utilization

Dear ______,

As you may be aware, our group at the Medical College of Wisconsin has been leading research on sport-related concussion over the past 20 years.   This work has only been possible through our collaborative efforts with the athletic training community in Wisconsin and around the United States.

As part of our ongoing engagement with you, we are now writing to you to request your participation in a brief survey.  The aim of this survey is to evaluate the awareness and utilization of current resources and practice patterns related to the concussion management and education among Wisconsin athletic trainers.  The survey is very brief and will only take about 10 minutes to complete.  Please click the link below to go to the survey Web site (or copy and paste the link into your Internet browser).

Your participation in the survey is completely voluntary. This survey will not ask for any direct identifiers, and the study team will not attempt to identify any subjects from their responses. Any published results would ensure that individuals could not be identified.

This survey has been approved by the Medical College of Wisconsin Institutional Review Board.  Should you have any comments or questions, please feel free to contact me at dhuber@mcw.edu or 414-955-7307.

Thank you very much for your time and feedback.

Sincerely,
